Requirements for Enrollment 

Old students

  1. Report Card

New Students

  1. Passing mark in the qualifying entrance examination.
  2. Original Birth Certificate (NSO)
  3. Photocopy of Baptismal Certificate
  4. Report Card (Form 138)
  5. Good Moral Certificate
  6. Immunization Records (Preschool only)
  7. Medical Certificate

Guidelines

  1. Take the Entrance Examination at the Guidance Center (second floor).
Bring the following:
- Photocopy of the report card
- 2 copies of recent 1x1 colored picture with white background
- Testing fee
  1. After a given number of days, call or visit the office for the result.
  2. If qualified, comply with the all requirements for enrollment.
  3. Parents or guardians MUST attend the scheduled orientation before classes start.
NOTE:
Age minimum requirements
  • Nursery - 3 years old
  • Kinder - 4 years old
  • Preparatory - 5 years old
  • Grade 1 - 6 years old (must be a reader)
