C. Enrollment Procedure 1. Student will present his/her student number, report card and (DS Form 2 for new student) to the registrar for verification of information. 2. Enrollment/assessment forms will be issued. 3. Enrollment and assessment forms are submitted to the treasurer's office for payment. 4. After payment, receipts are presented to the registrar to secure the admission card. 5. The admission card should be accomplished and presented to the class adviser on the first day of school. 6. New students who failed to submit the required credentials before the end of the enrollment period is considered not officially enrolled. 7. Credentials submitted for enrollment become property of the school and cannot be withdrawn after the registration, unless the student officially leaves the school. A request of the copy of the credentials can be accommodated within thirty days upon filing of such request. |
